MOS Burger is a Japanese fast food chain that shows how well it can be done. When I was over there they had 300+ restaurants serving freshly cooked fresh food including burgers that tasted of beef and chips (fries) that had recognisably once been part of a potato. They also served the drinks in actual glasses and even supplied cutlery. Of course, they were dwarfed by the likes of McDonalds and KFC, but they were only slightly more expensive and infinitely better.
 
Mr Donut is also big over there, and serves pretty good donuts and more or less drinkable coffee, the latter being a rarity in Japan.

Best Mexican fast food:  Chipotle.

The poll is also missing Subway, another favorite of mine, and though it might not technically be fast food, places like Panera Bread and Cosi serve much better food than McD/BK fairly fast, though at appreciably higher cost.

Other. In Finland there's Hesburger, which is almost the same as McDonald's, but the food is better in my opinion.

Don't eat that. You'll get a stomachache, or you'll get very fat and sick. Now, perhaps the quality of those services there is far better than here; but a friend ate a Burger at burger king (local version) and vomite for a week because of how old the meat was. Anyway, it's your life and your money. I cook for myself, because I love myself tremendously. Crostini is my way of life. Nothing can be as delicious as Crostinis...

Although I live in New Jersey, I've only been to White Castle once (was good actually).  I'd have to go with Arby's though, since their food taste more "real" than any other fast food place I've been to.
