I must confess being easily attracted to the work of Pushead, former singer of cult hardcore band Septic Death and creator of visuals for Prong, Metallica, etc. and also his own band.
I also dig Giger's work on ELP's "Brain Salad Surgery", Celtic Frost's "To Mega Therion" or Carcass' "Heartwork".
French drawer Philippe Druillet had created covers for interesting metal bands (Agressor, Sortilege, Proton Burst), William Sheller and Richard Pinhas (leader of Heldon).

Tough choice for me between Roger Dean and Hipgnosis.  The latter I think was more prolific.
Released date are often when it it impacted you but recorded dates are when it really happened...
