This is a good one. I'm sure I'm forgetting a bunch of people but these are the first ones to come to mind.
Geddy Lee
Greg Lake
Frank Zappa
John Entwistle
Steve Winwood
Bonus Pick: Paul McCartney (Not a virtuoso but very competent on bass, guitar, piano & drums)
Update, Can't believe I forgot Rachel Flowers.

Don't know about 'top five' but this lady is pretty amazing:

Seen her live with Dweezil Zappa and she plays guitar as well as keys, sax, flute and incredible vocals.

I dunno that I have a top five, but here are five that spring to mind:
Pekka Pohjola is mainly known as a virtuosic bass player, but was also a wonderful pianist and violinist. His first two solo albums showcase these talents.
Tony Duhig and Jon Field together made up the classic incarnation of Jade Warrior. Between them they played almost anything you can think of.
Jarrod Gosling is the singer, guitarist, bassist, keyboard player, drummer, accordionist, etc of the splendid Regal Worm.
Emma-Jean Thackray plays trumpet, bass, guitar, keyboards, vibraphone and bass clarinet - also sings. She probably plays a bunch of other things too that she didn't manage to fit onto the one album she's released so far.
We could probably do a separate thread on bands primarily made up of multi-instrumentalists. Gentle Giant would be the obvious prog example but there's lots more.

Cool topic... Could you take a minute to post the instruments they play, though? Cheers!
Steve Winwood (Hammond organ, acoustic and electric guitars, piano, harpsichord, bass, percussion, Mellotron, Moog)
Jimmy Page (electric, acoustic, lap steel and pedal steel guitars, theremin, banjo, bass, mandolin, Gizmotron)

How about the Shulman brothers. Andy Latimer as well. Ian Anderson is also a pretty good multi-instrumentalist as well as he can play acoustic guitar, flute, and possibly violin as well.

Jade Warrior didn’t always give instrumental credits, but on the Floating World album we have:
Tony Duhig / acoustic & electric guitars, bass, piano, organ, glockenspiel, vibes
Jon Field / Gaelic harp, gong, glockenspiel, vibes, alto, concert & Japanese flutes, congas, bell tree, African talking drum, cello, piano, organ, acoustic guitar
