Hello and welcome to the Annual Readers DPRPoll 2012! 
Once again we will have loads of great prizes to raffle amongst those of you who submit completed enties. We are still finalising this year's prizes, so please keep checking back regularly for the final list.
Please fill out this form and send it using the submit button at the bottom of the page. It takes a few moments for the data to submit to the database so please be patient and wait for the acknowledgement page. Be careful not to hit the 'return' button as this will submit your form with your entries incomplete. In case this happens just press 'back button' of your browser, finish your entry and submit again.
Everybody who fills out the completed form has a chance to win one of the great prizes. Only ONE entry per person and you must provide a first name & last name plus a valid email address. The entries from bounced confirmation emails will be deleted! People who deliberately send in multiple entries will have their entries disqualified. We will never give your email address or any other data you submit to anyone else.
Only 2012 originals count; all re-releases will be ignored in the analyses. Check the year of release / copyright on the CDs to see if it was released in 2012. We know that some albums were released over the Internet in December 2011, but had a general release in January 2012, so those will be allowed as well. Check the band's websites for the release date of such albums.
Please note that if you don't have answers to all questions, or don't have 10 favourite albums, you can leave a few of these blank at the end. 'Complete' means you take both the poll entry form seriously, so if you skip a few fields, your entry will still be accepted. Multiple entries for the same album in one category will be disqualified.
The Poll is open from now until Thursday, 31st January 2013. The results will be published on the DPRP site shortly after, as well as the names of the lucky winners.

Please use the TAB key to move through the form - do not use the ENTER key because this will submit your entry - if this happens you will have to click on the BACK button and then complete your entry. Duplicate entries are checked and the incomplete one will be deleted.
