I found him: Joseph Merrick (1862-1890) aka the Elephant Man. All the hints fit. I'll be back later with a new one.

That was the last Genesis album I bought. There are quite a few good songs on it, but it also showed very clearly the path they were choosing for the next decade.
It doesn’t look like Clement Dukes fits in (you don’t die from Dukes’ disease, and Clement himself isn’t prominent enough to have his own Wiki article).
