It depends how much self control you have and the environment you're in.

I smoke a lot when camping and I never get paranoia.

If I was smoking it on my own at home, then I might.  I have suffered from paranoia before many times whilst stoned but it does depend on the environment.

Last time I smoked I had a full blown panic attack and was thoroughly convinced that I was dying.
