I used the style from my Polytechinc instructions, looking like this:

 

...these most likely being contributions of HARRI PYSTYNEN, [Lehtonen] who had studied and played winds at the Finnish army orchestra [Esko Lehtonen: Suomalaisen rockin tietosanakirja, Soundi 1983.].

 
BTW,I was reading that BIO the other day and really enjoy the way he mixes personal observations with fact and info from other sources.

I think as long as due acknowledgement is always clearly given, the actual format is not too important.
