Your favorite transition from one song to another? I'm not sure if it's my favorite, but an example that comes to mind is the transition from "Sgt. Pepper's Lonely Hearts Club Band" into "With a Little Help From My Friends."

The transition between Buenos Tardes Amigo and HIV song on Chocolate and Cheese by WEEN. It goes from a ten minute epic tale of two brothers ending in the sound of horses galloping to carnival music with Ween yelling AIDS!

A transition I love a lot is from Prophet's Song to Love of my Life by Queen.
