Another one from the amazing book 1000 Record Covers by Michael Ochs:

I just find it funny that two different immediately identifiable Storm
Thorgerson covers are considered some of his werdrer ones. I think it's
obviously Storm, and neither one is particularly strange if you
consider his oeuvre.

The original of the above, predating LZ3 be a number of years, also had a rotating wheel
The original of the above (here, a version sadly mucked about with too much) found in the gatefold LP sleeve of Be Good To Yourself was special.

Hey guys, any album cover with Frank Zappa will be weird! My choice is "Abominog" (URIAH HEEP) - voted worst cover of the year when it was released. That devil monkey is weird - but ridiculous

Hot Rats was one of the first album covers i ever saw and for some reason even as a young 7 year old boy thought it was amazingly weird

Indeed. Gaboni made some very hilarious covers for Mago de Oz. One can find alot of "subliminal imgaes" in only one cover.

For me, it's Pink Floyd "Relics" cover with that four eyed mask. It makes me feel my eyes are going out of focus or I'm seeing double.

Oh, how could we forget this gem.
I always thought this one was quite strange, but cool.

Amputechture by TMV has a pretty weird cover, though very nice at the same time. My favourite cover ever.

Thes two are wonderfully weird imo.
Guru Guru: Hinten is probably the ugliest cover ever made. I won't post it.

Ulver: Kveldssanger. The hideous disaster of a cover looks like its
done by a twelve year old. The music is pure acustic folk songs. A very
pleasant surprise.

Not sure if it's the "weirdest", but Gaboni's drawing for the cover of MAGO DE OZ's album "Gaia" is quite bizarre:
